Electronic Application Form - Questions and Answers




Introduction

The Electronic Application Form is a computer program on a floppy disk. It displays the Charities Board's main grants application form, allows entry and storage of the application details, and the printing of an Application Form Declaration for signature.



Starting the Electronic Application Form disk

What sort of computer is needed?

The computer used must be a PC - not a Mac (Apple Macintosh). You need a 486 PC or higher, with a 3.5 inch floppy disk drive, running Microsoft Windows 3.1, 3.11, 95, 98 or 2000. It is possible to run it on some versions of Windows NT but you may find some strange effects on the screen.

How do I start the Electronic Application Form?

  1. Download the application form from the referring page.

  2. Insert a blank, virus free floppy disk into your PC.

  3. Double click the application form file and choose to "unzip". This will create an application form disk with all of the files needed for you to make an application.

  4. If you have Windows 3.1 or 3.11, click on File, Run and type A:\nlcbform then press Enter. (In some cases this may be drive B.)

  5. If you have Windows 95, 98, 2000 or NT, click on the Start Menu (move cursor to bottom left of screen), select Run and type: A:\nlcbform then press Enter. (In some cases this may be drive B.)

  6. Alternatively, in either case, you can use File Manager or Windows Explorer to find nlcbform.exe on the floppy disk (drive A: or in some cases it may be B:) and double click on NLCBFORM.exe (It may be shown simply as NLCBFORM).

  7. The first box to appear asks whether you are using a Screen reader or Braille software. Click 'yes' or 'no'. The Introduction to the application form will then appear on your screen followed by the form itself for you to fill in. The Introduction contains general guidance for using the software. It can be viewed again at any time, as can other guidance, from the Help menus.



Entering Data

Full instructions for entering data and moving around the application form are given in the Help menus on the disk. The following is a summary of the ways to move around the form:

  • Use Tab to move from one question to the next.
  • Use Back-tab (Shift +tab) to move from one question to the previous question.
  • Use up and down arrows to move up and down boxes in most tables (but not all).
  • Use Next from the menu bar or Page Down to move to the next page.
  • Use Back from the menu bar or Page Up to move to the previous page.
  • Use the Index on the menu bar to move to a specific section of the form. Note that the index has two levels - select the main section from the top level list and the second level will appear with the relevant sub-sections. Choose the required sub-section to move directly to it.

Why do the totals appear automatically in some tables but not in others?

Automatic totalling has only been activated for the large tables in section C19. Applicants must calculate the totals themselves in other parts of the form.



Printing Problems

Why do I need to print the form?

Applicants must print the Application form declaration (Section E) to be signed and submitted to the Charities Board with the completed disk. They may also need to print the Consortium pages (Section D) to send to their consortium members. Other parts of the form can also be printed but this is for the applicant's convenience only.

Why is printing from the disk so slow?

Printing from the disk uses a graphic image of each page, just like a screen print. It is therefore rather slower than normal printing and looks different.

Why does the screen keep jumping when I print?

The program prints a separate image of each page and as it progresses through the document you will see each page briefly moving on to the screen.

How do I print different parts of the form?

You can print the whole form, the Application form declaration, the Consortium pages, a single page or a selection of pages. Select 'File' + 'Print' on the menu and select the option that you want.

Cancelling printing

How do I cancel printing once it has started? Select 'Cancel' on the boxes that appear as each page is printed.

Screen reader and Braille display software

What is a screen reader?

It is special software that uses a voice synthesiser to speak what appears on the screen. It is used by visually impaired people.

What is the menu item 'Using Screen reader'?

It provides specially tailored help for people using screen readers and displays some parts of the screen in a way that interfaces better with some screen readers.

Does it work with all screen readers?

The screen reader mode works best with the 'JAWS' screen reader. Other screen readers may work better with screen reader mode switched off. (It can be switched on and off at any time from the File menu.) You have to experiment. It may not be practical for some screen readers.

How do I switch the special screen reader mode on and off?

You normally switch on the special facilities that operate with Braille display or screen reader software in the first dialogue box when you start up. You can switch them on or off at any time by selecting 'File' and 'Using screen reader' from the menu bar.



The Application Form Declaration

What is the Application Form Declaration ?

The Application Form Declaration is Section E of the form and is identical to that part of the paper form plus some additional instructions. Although it can be filled in on screen, it must be printed out in order for it to be signed by several different people.

How do I make a last minute change to the answers - I have already printed the Application Form Declaration?

Changing the answers will change the Application Validation Number that is recorded on the disk and printed on the declaration. You must re-print the declaration and get it signed, after changing the answers.

What is the Application Validation Number for?

It allows us to check that the Application Form Declaration matches the answers you gave on the questions and that you have not made any changes to the answers after printing the form.

How can I tell if the answers have changed since I printed the declaration?

Look on the screen on the first page of the Declaration. It shows the Application Validation Number. If this is the same as the one on the printed declaration, the answers are unchanged.



Copying the Disk

Why should I copy the disk?

Copying the disk is important. It is a safety measure in case the disk gets lost in the post or is unreadable when received

How do I copy the disk?

1) Place the disk in the drive

2) Select File Manager or Windows Explorer

3) Right click '3 �" floppy' and select 'Copy disk'



Making room for extra or longer answers

What is the Copy button for?

On certain parts of the form, the Copy button allows you to duplicate a set of questions, in places where they need to be repeated.

How do I copy a set of questions that I need to repeat?

Click on the Copy button if there is one. You can do this more than once if necessary. Press Page down to move to the copied questions. Note that they are on a separate page with the same page number. If there is no Copy button, the questions should not need duplicating.

There is not enough space for my answer

If we have not provided enough space for a particular answer you may continue in the continuation box at the end of Section D. The box can be duplicated if necessary but do not extend any answer into more than one box. Make sure you tell us which question you are answering in the continuation box.



Disk Corruption

When I try to start the form I get a message saying that the file appears to be corrupt.

Have you altered any files on the disk with any other software than that provided?

What did you do prior to getting this message. (eg Copy, Save)?

Saving Data and Re-Loading

How do I save my answers?

You can save your answers at any time. They will be stored on the floppy disk. When you close the form, you will be asked whether you want to save your answers if you have made any changes since the last save. To save at any other time, select 'File' + 'Save' from the menu bar.

How do I get my answers saved automatically?

You can save at regular 5 minute intervals. Select 'File' + 'Autosave every 5 mins' from the menu bar.

How do I load up previously stored answers?

Answers that were previously saved will be re-loaded automatically when you start the form.

If you want to discard changes made since you last saved you can re-load the last set of saved answers by selecting 'File' + 'Re-load' from the menu bar.



Lost Data

The data I entered last time has disappeared

Did you save before or on the point of closing? If not, your data will be lost.

Did you turn off the computer in the middle of working?

  • Yes - If no save was made the data will be lost and you will have to start again. Next time, try using 'Autosave every 5 mins' from the File menu.

Was there a crash or freeze of your system?

  • Yes - this would lose any data entered since the last save. Next time, try using 'Autosave every 5 mins' from the File menu.

Was the data in a large text box?

  • Yes - It is possible to enter slightly more data in large text boxes than the boxes should allow. The extra text will not be saved and only visible text will be printed. Use the Continuation box at the end of Section D to extend answers that do not fit into the normal answer boxes.



System stopped or frozen

My System has stopped and I cannot do anything with the form

Is anything else running on your PC at the moment?

  • Yes - please close down everything other than the application form and try to start it in the normal manner. If it does not start remove disk, restart the computer and start Electronic Application Form again.

  • NO - try Ctrl+Alt+Del once (or Ctrl+Esc in Windows 3.11) - do you get Close Program Window? If yes, then highlight nlcbform and click on End Task.
    Re-start the Electronic Application Form.



Miscellaneous

Why do I have to keep clicking on boxes that say OK?

Because you have got the screen reader mode switched on. Switch off 'Using Screen Reader'
